Collaborative map with road and bridge conditions

GIS genius Don Meltz has created a collaborative map of road conditions in the Hudson Valley and the Catskills. It is amazing.

We were having problems because the spreadsheet was getting high traffic. So here's a form that (hopefully) will solve that problem.
